We need your help to research client attitudes to therapy. Our aim is to gain a deeper understanding of the way people engage with therapy and the various factors that are important to people undertaking therapy. We are grateful for the time you have taken to share your opinions.
All the forms are anonymous and the information is confidential. All forms will be stored and accessed in accordance with GDPR and the international standard for good clinical practice within research. Our privacy and data policy can be found below.

Curated Self “Privacy” and “Special Category” Policy
This notice applies to all visitors to our website and anyone who requests therapy or takes part in our research process as a participant. It explains how our organization collects, processes, stores your information. You can request the identity of the data controller, if requested.
1. What type of information do we collect and process?
1.1 For therapy services:
Name, Address and contact details (email and telephone numbers)
Doctor’s name and address
Date of birth
1.2 For research purposes:
Anonymized data
We may also collect, store and use more sensitive personal information referred to by the ICO as “Special categories” information. Particularly sensitive personal information requires higher levels of protection. We need to have further justification for collecting, storing and using this type of personal information for the purposes of providing suitable therapy services or research purposes. We may only process special categories of personal information with your explicit consent.
2. What type of special categories information do we collect and process?
Information about your health, including medical conditions, health and sickness records.
Ethnicity information.
3. How we collect your data?
Most of the personal information we process is provided to us directly by you:
when you complete an online submission form to request therapy or participate in a research study.
the information you submit is imported onto secure servers and encrypted in real time.
we may also receive additional personal information directly by telephoning you if required.
we may also receive additional personal information through your GP, Psychiatrist, Psychologist, or other allied health professionals
4. How will we use your data?
We use the information that you have given us in order to:
evaluate your request for therapy services
process the data submitted as part of a specific research study
to improve therapy services
5. How we store your data?
Your information is encrypted and securely stored in the cloud on servers in the UK.
We keep personal information for 3 years. We will then ask you if you wish your information to be removed securely thereafter.
6. What are your data protection rights?
Under data protection law, you have the following rights including:
Your right of access – You have the right to ask us for copies of your personal information. We may charge you a small fee for this service.
Your right to rectification – You have the right to ask us to rectify information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.
Your right to erasure – You have the right to ask us to erase your personal information in certain circumstances.
Your right to restriction of processing – You have the right to ask us to restrict the processing of your information in certain circumstances.
Your right to object to processing – You have the the right to object to the processing of your personal data in certain circumstances.
Your right to data portability – You have the right to ask that we transfer the information you gave us to another organisation, or to you, in certain circumstances.
